mental health benefits verification. Services may be covered in full or in
part by your health insurance or employee benefit plan. Please check your
coverage carefully by asking the following questions: Do I have mental
health insurance benefits? What is my deductible and has it been met? How
many sessions per year does my health insurance cover? Do I have
out-of-network provider benefits (when applicable)? If so, do I have to meet
a deductible and/or what is the reimbursement amount? Are LMFTs (Licensed
Marriage and Family Therapists) on the list of approved providers? Is
approval required from my primary care physician?
The following insurances reimburse in-network:
Lifetime Benefit Solutions (Formerly EBS-RMSCO)
Check with your insurance for additional in-or-out of network coverage
information. Other businesses and insurances may reimburse that are not
mentioned above.
For clients who are pursuing out-of-network coverage, or are paying out of
pocket, we ask that payment be made at the time of each appointment; credit
card, FSA card, check or cash is accepted.
Reduced Fee
Reduced fee services are available on a limited basis.
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have
not notified us at least 24 hours in advance, you will be required to pay a
$100 fee. Some exceptions may be considered for emergency, illness and
weather-related cancellations.
